Notice to Texas Consumers
In accordance with Texas law (House Bill 4224, 89th Regular Session, and Section 181.105 of the Texas Health & Safety Code), licensed behavioral health providers in Texas must provide certain consumer information. This page explains your rights and how to exercise them.
Your Rights as a Consumer
As a client of D2 Counseling, you have the right to:

Request access to your health care records that are maintained by this practice.

Contact the Texas Behavioral Health Executive Council (BHEC) regarding licensed behavioral health providers.

File a consumer complaint with the Office of the Texas Attorney General’s Consumer Protection Division.
Requesting Your Health Care Records
You may request copies of your health care records that we maintain.

Filing a Complaint with the Texas Behavioral Health Executive Council
If you have concerns about the conduct of a licensed behavioral health provider in Texas, you may contact:

BHEC oversees licensing boards for professional counselors, marriage and family therapists, psychologists, and social workers in Texas.
Filing a Consumer Complaint with the Texas Attorney General
Texas consumers may also file a complaint with the Office of the Texas Attorney General’s Consumer Protection Division if you believe you have been harmed by a provider’s professional conduct. Current instructions for filing a consumer complaint are available on the Attorney General’s website.
